     The purpose of Contract Health Services (CHS) is to assist our Native American patients with needed medical services not available within our healthcare system.
     The patients' Choctaw Nation physician determines a medical service is needed and fills out a patient referral notice and directs the patient to the CHS office. The CHS staff will counsel the patient and explain the CHS guidelines and possibly schedule an appointment with an outside provider.
     Persons who require emergency care at another facility and were not able to make it to our facility must notify CHS within 72 hours after the beginning of treatment. This is called an emergency room call-in and this is just to notify CHS that you received treatment. Medical records have to be obtained and presented to the CHS Office for determination.
     All referrals and emergency room call-ins for eligible patients are taken before the CHS Medical Review Committee and ranked according to medical priority for "life threatening or life disabling" conditions. From this ranking and availability of funds, CHS determines approval or denial of funds for the patient.
     Those patients that are approved for funding will have appointments scheduled and will have their care authorized through CHS. Those patients that are denied funding can have an appointment scheduled but will be responsible for payment of their care.

ELIGIBILITY CRITERIA
     1.    Must be a member of a federally recognized tribe and reside within the Choctaw Nation CHS Service Unit. This comprises the counties of Atoka, Choctaw, Haskell, Latimer, Leflore, McCurtain, Pittsburg, and Pushmataha.
     2.    Must obtain a patient referral notice for every visit outside of our healthcare system.
     3.    CHS is the "payer of last resort", meaning all other resources have to be obtained and must be utilized for your care. Title-19, Short-term Medical Assistance through the Department of Human Services must be applied for and a determination made if you do not have a third party payer.
