Job Description

Works under the general supervision of the CRC Manager providing proactive case management services for the outpatient clinics. Ensure services within the CNHCC are utilized whenever possible before referrals are made outside the system. Provide follow up from hospitilazations and outside physician appointments. Provide appropriate reimbursement information to physicians regarding individual patients. Assess and address case management needs of individual patients. Responsible for the direction and evaluation of patient care delivery. Assist with Disease Management within our health care system. Provide reports regarding care delivery whenever requested.

Qualification

Graduate of an accredited school of nursing with current RN or LPN licensure. Minimum of 2 years of current accute nursing experience. Requires ability to handle stressful situations. Dietary Aide Level 1 is the entry level position in the Dietary department. Performs a variety of food preparation duties, such as preparing cold foods and beverages for patient and cafeteria service. As scheduled will work in the kitchen and cafeteria areas.
